NPs are unique health care providers as registered nurses with graduate education and the legal authority to diagnose, order lab tests, and prescribe medications. No other health care role has been legislated, regulated, or scrutinized to the extent of the NP - the evidence is clear with positive patient health outcomes thus cost reductions to the healthcare system.

Nursing science is a topic students and faculty view with skepticism. The interchangeable terms and the need to understand the contextual nature of nursing philosophy lead to feelings of anxiety and confusion. Students question the relevance of learning nursing science in relation to practice. The analogy of a building is used to explain how nursing philosophy relates to practice. The analogy of interrelated concepts may help students gain an understanding of nursing science, which is essential to further knowledge development in nursing.

•Anxiety and depression are frequent complaints in primary care. •Meditation or mindfulness strategies may help decrease health care costs and improve health outcomes for anxiety and depression. •In addition to Buddhist philosophy, the 2 main secular interventions of Western mindfulness are mindfulness-based stress reduction and mindfulness-based cognitive therapy. •Mindfulness can be cultivated through a variety of practices, including meditation, body recognition, and yoga. •Nurse practitioners must ensure patient suitability before the initiation of mindfulness-based intervention.

•Bimanual examinations are not an effective screening tool in asymptomatic well women. •The bimanual exam is not recommended for cancer screening, sexually transmitted infection testing, or prior to initiation of contraception. •Clinicians continue to perform the bimanual exam despite evidence-based rationale as to its futility. •The greatest harm generated by pelvic exams is the detection of false-positive pathology.

Biography

Els Duff, NP, PhD is a nurse practitioner (NP) and nurse educator. Her understanding of the NP role from practice, education, and regulation has provided insight for regulation and education. As a NP scientist, Duff works collaboratively with interdisciplinary teams to conduct research focusing on NP education, practice and competence. Primarily through the use of web-based national survey quantitative methods, her research explores the perspective of the NP in delivering health care services. She currently practices as a NP in a community setting. Duff was the recipient of the prestigious Queen Elizabeth II Diamond Jubilee Medal and the Canadian Nurses Association Ann Beckingham Award. Duff has served as a board member with the College or Registered Nurses of Manitoba and the Nurse Practitioner Association of Manitoba. As a member of the International Council of Nurses Nurse Practitioner, Advanced Practice Nurse Network research committee, Duff is currently engaged with nurses from around the world to revise and publish ICN advanced practice competencies. She teaches graduate level courses and mentors nurse practitioner students scholarly submission for peer reviewed publications and posters.
